Registered Nurse (RN), Clinical Oversight and Staff Training

About the role

The Registered Nurse provides clinical assessment, care planning, nursing oversight, staff training, consultation, and quality assurance for individuals receiving community-based services through Aya Living. This position is primarily focused on clinical judgment, oversight, education, and coordination rather than routine bedside nursing, private-duty nursing, or continuous in-home shift care.

The RN works closely with LVNs, Direct Support Professionals, supervisors, families, healthcare providers, and interdisciplinary teams to support safe implementation of physician orders, individualized health plans, and client-specific healthcare needs. Direct nursing intervention may occasionally be required when clinically appropriate; however, routine medication administration, wound care, treatments, and other ongoing direct-care tasks are not expected to be the primary function of the position.

Responsibilities

• Conduct nursing assessments for clients with restricted health conditions, complex medical needs, or other significant healthcare needs.
• Review diagnoses, medications, physician orders, treatment instructions, hospital discharge information, and other clinical records.
• Develop and update individualized nursing care plans, health protocols, monitoring instructions, and staff guidance.
• Identify health-related risks and recommend appropriate safeguards, monitoring practices, clinical follow-up, and service modifications.
• Provide clinical direction, consultation, and support to LVNs assigned to agency clients.
• Review and clarify physician orders, treatment protocols, medication instructions, nursing plans, and documentation requirements.
• Monitor implementation of client-specific nursing recommendations and provide coaching or follow-up when concerns are identified.
• Support LVNs in recognizing, documenting, and appropriately responding to changes in client condition.
• Provide health-related training to LVNs, Direct Support Professionals, Team Leads, Coordinators, and other agency staff.
• Provide client-specific instruction regarding diagnoses, medications, restricted health conditions, treatments, precautions, emergency procedures, and documentation requirements.
• Evaluate and document employee understanding and competency for assigned health-related responsibilities.
• Provide refresher training, coaching, and corrective instruction when needed.
• Develop practical training materials, clinical protocols, competency tools, and staff reference materials.
• Review medication records, health documentation, incident reports, changes in condition, physician recommendations, and other information requiring nursing review.
• Follow up on medication concerns, unclear or expired orders, hospitalizations, emergency department visits, treatment issues, and discharge instructions.
• Identify documentation deficiencies, conflicting clinical instructions, unsafe practices, or gaps in implementation and coordinate corrective action.
• Participate in health-related incident reviews and recommend appropriate preventive or corrective measures.
• Collaborate with clients, families, physicians, pharmacies, therapists, Regional Center personnel, case managers, agency leadership, and service teams.
• Participate in assessments, care-planning meetings, service transitions, incident reviews, and interdisciplinary discussions as clinically appropriate.
• Assist with continuity of care following hospitalization or other healthcare transitions.
• Communicate clinical recommendations clearly and help service teams distinguish between nursing and non-nursing responsibilities.
• Support quality assurance activities involving restricted health conditions, medication systems, clinical documentation, staff training, and health-related risk management.
• Review health-related records for accuracy, completeness, and consistency with current physician orders and nursing plans.
• Identify health or safety concerns requiring corrective action or additional clinical review.
• Serve as a clinical resource to agency leadership regarding nursing policies, procedures, training standards, and service-delivery safeguards.
• Maintain accurate and timely documentation of nursing assessments, care plans, consultations, trainings, competency reviews, recommendations, and follow-up.
• Document clinical decisions and communications clearly enough to provide direction to the service team and support continuity of care.
• Maintain organized clinical records and protect confidential health information in accordance with agency policy and applicable privacy requirements.
• Conduct scheduled visits to client homes and community-based service locations throughout San Diego County.
• Complete in-person assessments, observations, competency reviews, trainings, and clinical follow-up when appropriate.
• Observe service environments and identify health-related risks or implementation concerns.
• Attend occasional in-person meetings, trainings, or clinical reviews at agency or community locations.
• Maintain reliable transportation for local travel.
