Registered Nurse (RN) - Chronic Care

Responsibilities

  • Conduct patient assessments and wellness visits in the home
  • Perform medication pre-pours and monitor patient status
  • Develop and update care plans
  • Care coordination with patients, families, caregivers, and providers
  • Supervise CNA and LPN services as required
  • Identify changes in condition and communicate concerns appropriately
  • Support patients in maintaining independence and quality of life

Requirements

  • Current Connecticut RN license
  • Home health, community health, chronic care, or case management experience preferred
  • Strong assessment and communication skills
  • Reliable transportation and willingness to travel throughout the service area
  • Current CPR certification

The pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.

Schedule

A 30-hour work week (Monday-Thursday), 4-day work week
