Responsibilities
- Conduct patient assessments and wellness visits in the home
- Perform medication pre-pours and monitor patient status
- Develop and update care plans
- Care coordination with patients, families, caregivers, and providers
- Supervise CNA and LPN services as required
- Identify changes in condition and communicate concerns appropriately
- Support patients in maintaining independence and quality of life
Requirements
- Current Connecticut RN license
- Home health, community health, chronic care, or case management experience preferred
- Strong assessment and communication skills
- Reliable transportation and willingness to travel throughout the service area
- Current CPR certification

The pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
Schedule
A 30-hour work week (Monday-Thursday), 4-day work week
