Registered Nurse (RN), Case Manager Hospice

About the role

Location: Kansas City, MO & Surrounding Areas

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life. Responsibilities

• Provide compassionate, hands-on hospice care
• Support and educate patients and families
• Complete assessments and ongoing evaluations
• Collaborate with physicians and the hospice team
• Address patients' physical, emotional, and comfort needs

Requirements

• Active Kansas or Compact RN license
• Hospice experience preferred — but we will train the right nurse
• Valid driver's license and auto insurance
