Registered Nurse (RN) Case Manager - Hospice

We are a post-acute care provider dedicated to the growth and development of our team. We CARE for our patients like they are our own FAMILY.

Schedule

• Monday- Friday 8:00am-4:30pm
• 1 Weekend day every 4-6 weeks
• Rotating Holidays as required

Akron, Kenmore and Surrounding Areas

What You Will Love Doing

• Through a thorough assessment, work closely with the patient’s attending physician and the hospice Medical Director to ensure every patient’s symptoms are well managed.
• Evaluate the treatment and medical condition of the patient according to the plan of care and adjust as needs change.
• Manage interdisciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Oversee and supervise total care of patient provided by nurse aides and LPNs.
• Attend in-service training and mandatory agency meetings.

• Patient with attention to detail and observant.

Requirements

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
