Responsibilities
- Participates in multi-disciplinary rounds to review patient status, progress, and discharge plans.
- Educates patients and families on the emotional, social, and financial impacts of illness and mobilizes resources.
- Assesses readmitted patients for reasons of readmission and organizes care conferences with the multidisciplinary team.
- Documents discharge planning evaluations, ongoing assessments, and discharge plans.
- Communicates with payors for authorization of post-acute care and assesses patients' and families' holistic needs.
- Develops discharge plans with contingency plans and coordinates post-acute care services and facilities.
- Leverages technology to communicate with post-acute care services and ensures accurate discharge reconciliation.
- Manages multiple tasks and prioritizes responsibilities to ensure timely care coordination and progression of care.
- Works collaboratively with the team and leverages conflict management and resolution skills.
Requirements
- Associate's degree in Nursing [Required]
- Bachelor's degree in Nursing [Preferred]
- 2+ years of medical/hospital nursing experience [Required]
- Prior experience in Care Management/Utilization Management [Preferred]
- Registered Nurse (RN) [Required]
- Certified Case Manager (CCM) [Preferred]
- Accredited Case Manager (ACM) [Preferred]

Schedule
The schedule is 36 hours per week, Monday through Friday, from 8:00 AM to 8:30 PM, with every other Saturday and rotating holidays.
Pay
The pay range for this position is $35.19 - $62.17 per hour.
