Responsibilities
- Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers and discharge.
- Monitors patient and family satisfaction; responds to questions and complaints.
- Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
- Performs chart reviews.
- Accurately documents case management activities, plans and outcomes.
- Supports department operations by working alternate shifts/units as needed; may be assigned on-call.
Requirements
- Active Virginia RN license (single-state) or compact license; if relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.
- Basic Life Support certification required within 30 days of hire.
- Previous RN/Case Management experience preferred.
- 3 years of RN clinical experience preferred.

Pay
Hourly range: $33.19 - $44.81
