Patient Care

  • Provides care in accordance with the Nurse Practice Act and professional nursing standards.
  • Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history.
  • Develops, implements, and revises the Plan of Care based on ongoing patient evaluations.
  • Administers medications and treatments as prescribed in the physician’s plan of care.
  • Educates patients and families on self-care, medications, and diet as appropriate.
  • Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition.
  • Collaborates with physicians, interdisciplinary teams, and community health partners to coordinate care.
  • Safeguards the environment for patients and supports their activities of daily living.
  • Provides emotional support and counseling to patients and families.
  • Arranges for necessary medical equipment and supplies.
  • Ensures HIPAA compliance and patient confidentiality.

Additional Duties

  • Participates in on-call visits as required including evenings and weekends.
  • Attends interdisciplinary group meetings.
  • Supervises ancillary personnel and delegates responsibilities as needed.
  • Engages in professional development through training and continuing education.
  • Contributes to quality assessment and performance improvement activities.
  • Performs other duties as assigned.

Qualifications

  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR).
  • MN, MI, and IL are not in the Nursing Licensure Compact.
  • Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.
  • Ability to pass DHS background, consumer report, and drug test.
  • Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.
  • 1+ years of recent experience in hospice/home care preferred.
  • 2+ years of Nursing experience, preferred.
  • Physical Requirements: Pushing/Pulling and Lifting/carrying up to 30 pounds. Occasionally up to 50 pounds. Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
