Registered Nurse RN Case Manager

Make a Meaningful Impact. Advance Your Career. Vibra Permanent Staffing is seeking an experienced and dedicated Registered Nurse Case Manager (CM) for a direct hire, full-time opportunity with one of our trusted Long-Term Acute Care (LTAC) partners.

Responsibilities

• Provide direct care to critically ill or medically complex patients
• Collaborate with physicians and an interdisciplinary team to coordinate individualized care plans
• Maintain compliance with all policies, standards, and regulatory guidelines
• Monitor patient progress, administer medications and treatments, and document clinical findings
• Educate patients and families, and serve as an advocate for long-term health and recovery

Qualifications

• Active RN license in position state, or compact state
• 1+ year of Case Management experience strongly preferred
• BLS required; ACLS required within 90 days of hire
• Strong clinical judgment and a commitment to excellence in patient care
• Compassionate, team-oriented mindset
