Job Summary

The Registered Nurse RN Case Manager role at Chippenham Hospital involves supporting patients and families through their care journey. Key responsibilities include coordinating services, developing care plans, and facilitating patient/family/caregiver education.

Qualifications

  • Graduate of an accredited school of nursing (BSN preferred)
  • Current Registered Nurse license in Virginia or current Multistate Registered State license
  • 3+ years of RN experience in a hospital setting preferred
  • Previous case management or utilization review experience preferred

What You Will Do

In this role, you will:

  • Manage patients' care from admission to discharge
  • Collaborate with the health care team to develop treatment goals and plan interventions
  • Document patients’ social work assessments, treatment goals, interventions, outcomes, and discharge plans
  • Provide program orientation to patients/families/caregivers
  • Promote patient/family/caregiver participation in team discussions related to plans, goals, and status
  • Ensure the implementation of the patient's treatment plan that supports their strengths, abilities, needs, and preferences
  • Facilitate the involvement of the patient throughout the rehabilitation process
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments
  • Communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service
