About the role
Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.
Responsibilities
- Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.
- Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
- Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
- Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
Requirements
- Active Michigan RN license
- Reliable transportation, valid driver's license, auto insurance
- Home health / OASIS experience strongly preferred
Qualifications
- Dedicated clinician: home health experience strongly preferred

Skills
- Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge)
- Direct, high-acuity patient care, with a focus on wound care
- Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases
- Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
Benefits
- Full-time target: ~30 productivity points/week; clinicians earn more for exceeding it
- Guaranteed floor: ~21 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor.
Pay
Compensation range: $85,000-$105,000 approximate yearly pay
Schedule
Full-time Monday–Friday + some weekends (as needed)
