About the role
The Registered Nurse (RN) Care Team Case Manager (CTCM) at One Community Health (OCH) provides services in support of medical providers and the clinical care team. Within the scope of licensure and responsibilities, the RN CTCM delivers care coordination, case management, and acute interventions following established protocols. This position also supports patients through transitions of care and across the care continuum to promote high-quality, patient-centered outcomes.
Responsibilities
- Delivers care coordination, case management, and acute interventions following established protocols
- Serves as a liaison between patients, families, and the clinical care team to ensure continuity of care
- Assesses patient needs and develops care plans to address health issues and improve outcomes
- Coordinates referrals and transitions of care among various healthcare settings
- Collaborates with interdisciplinary teams to develop and implement care plans
- Monitors patient progress and adjusts care plans as needed
- Provides education and support to patients and their families regarding health conditions, medications, and self-care
- Documents patient interactions and care activities accurately and timely
- Participates in quality improvement initiatives and performance monitoring
- Maintains confidentiality and adheres to HIPAA regulations
Requirements
- Graduate of an accredited nursing program
- Current Washington State Registered Nurse license
- Minimum two (2) years of RN experience; preferred experience in ER, triage, or care coordination/case management
- Experience in adult and pediatric primary care
- CPR/Basic Life Support (BLS) certification
- Valid driver’s license and proof of auto insurance
- Strong written and verbal communication skills
- Ability to communicate clinical concepts effectively to both clinical and non-clinical audiences
- Ability to prioritize and organize work across multiple and sometimes competing deadlines
- Ability to meet team and organizational goals with a high level of quality and professionalism
- Ability to inspire trust and confidence while communicating effectively with internal and external audiences
- Strong computer skills, including Microsoft Office applications, Epic, and other systems
- Knowledge of and compliance with HIPAA regulations
- Knowledge of customer service and workplace safety practices
- Ability to remain calm and effective in high-pressure or emergency situations
- Ability to perform and operate medical equipment, including venipuncture and IV skills
Preferred Qualifications
- Bilingual in English and Spanish
- Experience working in a Federally Qualified Health Center (FQHC) or look-alike organization
- Experience in an integrated, team-based care setting
- Experience using Epic Electronic Medical Records
- Familiarity with a variety of healthcare concepts, practices, and procedures
- Experience as a supervisor or team lead in healthcare or a related setting

Compensation
Hourly/Non-Exempt: $38.00-40.00 per hour, depending on experience
Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
