Registered Nurse RN Care Manager Hospice
About the role
Provides hospice nursing services to patients at home or in nursing facilities. Coordinates and oversees patient care performed by Certified Nursing Assistants. Conducts patient assessments and provides skilled nursing care. Makes recommendations and collaborates with physicians. Utilizes clinical nursing skills to manage patient caseload per hospice policies and regulatory standards. Provides comfort and support to family members and caregivers. Educates families, caregivers, and patients on end-stage disease processes, safety issues, and symptom management. Participates in interdisciplinary team meetings. Orders necessary resources and supplies, including medications, supplies, and medical equipment. Completes all required documentation in the electronic medical records system. Serves on-call with other team members for team emergency clinical needs after-hours.
Responsibilities
- Communicate by reading, writing and typing legibly, speaking and comprehending English effectively in order to carry out job requirements.
- Knowledge of electronic medical record.
- Knowledge of nursing skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee.
- Knowledge of basic to intermediate application of Microsoft Office Suite including, Word, Excel, PowerPoint, Outlook as well as internet software.
- Ability to read and interpret documents such as physician orders, plan of care documents, policies and procedures, and other instructions furnished in written, oral, diagram, or schedule form.
- Ability to follow standard precaution guidelines for infection control, including when providing patient care.
- Ability to work independently and manage multiple tasks with prioritization of their importance.
- Demonstrates critical thinking and problem-solving skills.
- Demonstrates effective organizational and customer service skills.
- Work with people of all social, economic, and cultural backgrounds; be flexible, open-minded, and adaptable to change.
- Knowledge of assessment competency and application for all ages served within the long-term acute care environment.

- Knowledge of the insurance industry and government reimbursement for healthcare settings.

- Computer proficiency with Outlook email and electronic medical records.

- Understanding of CMS Conditions of Participation for Discharge Planning.

Requirements
- Associate's of Nursing [Required]
- Bachelor's of Nursing [Preferred]
- Registered Nurse (RN) [Required]
- Basic Life Support - CPR Cert (BLS) [Required]
- Driver's License (DL) [Required]
- Auto Insurance (AL) [Required]
Qualifications
- 2+ years of hospital nursing experience [Preferred]
- Specific experience in Case Management and discharge planning. [Preferred]
