Responsibilities

• Patient Population (Clinical Only):
  • Engages in population-appropriate communication.
  • Has knowledge of growth and development milestones and tasks.
  • Gives clear instructions to patients/family regarding treatment.
  • Involves family/guardian in assessment, initial treatment, and continuing care.
  • Identifies physical limitations of the patient and deploys intervention when necessary.
  • Recognizes and responds appropriately to patients/families with behavioral health problems.
  • Interprets population-related data and plans care appropriately.
  • Identifies and responds to unique psychological needs or those associated with religious/cultural norms.
  • Performs treatments, administers medication, or operates equipment safely.
  • Recognizes and responds to signs/symptoms of abuse or neglect.
• Patient Assessment & Interventions:
  • Acts as an expert clinician and role model in providing quality patient care within specialty area.
  • Utilizes the nursing process for direct patient care and assists staff with assessment, planning, implementation, and evaluation of patient care needs.
  • Conducts thorough assessments of patients’ health status during clinic visits and reassesses as needed.
  • Documents nursing history and assessment, initiating referrals based on patient needs.
  • Collects basic and advanced patient data; monitors for trends impacting patient health.
  • Maintains awareness of comfort and safety needs of patients during ambulatory appointments.
  • Assesses patient environmental safety continuously.
  • Responds to life-saving techniques based on nursing standards, policies, procedures, and protocol.
  • Provides accurate hand-off reports when patients move between ambulatory and inpatient areas.
  • Recognizes and reports changes in patient conditions and abnormal labs or results.
  • Performs point-of-care (POC) testing assessments as needed.
  • Initiates and implements individualized patient care plans in collaboration with healthcare providers.
  • Collaboratively develops and utilizes patient care pathways for consistent quality of care.
  • Demonstrates competency in administering medications, wound care, and other direct nursing interventions.
  • Completes nurse visits and documents timely and accurately.
  • Assists with minor procedures and diagnostic tests, ensuring patient comfort and safety.
  • Monitors patients for changes in condition, reports findings, and adjusts care plans accordingly.
  • Recruits patients for studies.
  • Assists and collaborates with staff nurses in quality assurance.
  • Meets new patients to introduce self and explain role.
  • Establishes priorities of care based on individual patient assessments.
  • Demonstrates critical thinking and accountability in evaluating and implementing physician orders.
  • Ensures timely and accurate nursing documentation according to hospital policy.
  • Maintains real-time nursing documentation.
  • Maintains a positive, professional, and cooperative demeanor in all interactions.
  • Remains calm during emergent or challenging behavioral situations with patients.
• Patient Education:
  • Meets with patients and families to identify educational needs related to health promotion, maintenance, and disease processes.
  • Educates patients and families about health conditions, treatment plans, medications, and self-care techniques.
  • Reassesses for understanding and re-educates as needed.
  • Applies current nursing literature in teaching techniques and guiding standards of care.
  • Serves as an expert patient educator for disease-based and clinic-based care teams.
  • Documents patient/family/designee teaching and response appropriately.
• Facilitation of Care:
  • Facilitates seamless transitions of care by coordinating referrals, scheduling follow-up appointments, and ensuring timely access to services.
  • Communicates effectively with patients regarding test results, treatment plans, and follow-up care.
  • Addresses patient concerns and provides support and guidance throughout their treatment journey.
  • Anticipates changing needs of patients and families based on care plans to reduce delays and financial toxicity while increasing satisfaction.
  • Participates in interdisciplinary team meetings to discuss patient care.
  • Collaborates with social workers, dietitians, and other healthcare professionals to address comprehensive patient needs.
  • Serves as a liaison to other departments and organizes interdepartmental projects.
  • Participates in transitions of care planning.
  • Documents plan of care reflective of collaborative practice with the healthcare team.
  • Supports prior authorization for medication and diagnostic tests.
• Leadership & Professional Development:
  • Serves on nursing, hospital-wide, and unit-based committees influencing patient care delivery.
  • Assists with developing and revising policies and procedures for nursing practice.
  • Assists with implementing evidence-based practice in the unit.
  • Works collaboratively on performance improvement system-wide.
  • Demonstrates accountability, responsibility, and commitment regarding patient care standards.
  • Serves as a resource within specialty for all members of the healthcare team.
  • Participates in orientation and other education programs as assigned.
  • Serves as a preceptor and/or mentor for nursing students, clinical interns, and other healthcare professionals.
  • Identifies opportunities for observation or clinical experiences.
  • Remains current in research related to clinical nursing practices.
  • Participates and contributes to staff meetings.
• Compliance & Safety:
  • Maintains confidentiality of information.
  • Sustains an understanding of regulatory requirements and accreditation standards.
  • Monitors safety and departmental policies and procedures.
  • Works with department managers, nurses, and multidisciplinary representatives to identify and share safety best practices.
  • Demonstrates an understanding of legal and regulatory issues (HIPAA, EMTALA, regulatory agencies, UC policies) impacting care delivery and reimbursement.
  • Utilizes MIDAS effectively to report safety concerns.
  • Able to escalate appropriately.

Requirements

• Minimum Required:
  • Associate's Degree in Nursing.
  • Current Registered Nurse (RN) license in the state of Ohio.
  • Minimum of 2 years of nursing experience in an ambulatory or outpatient setting.
• Preferred:
  • Bachelor's Degree in Nursing.
  • Basic Life Support (BLS) certification.
