About the role
Coordinates patient care for assigned patients utilizing the nursing process.
Responsibilities
- Utilizes the nursing process of assessing, planning, implementing, and evaluating all nursing care provided within the Cardiac and Pulmonary rehab settings.
- Demonstrates knowledge of exercise principles and development of an appropriate exercise/rehab plan based on cardiac and pulmonary rehab patient evaluation and assessment.
- Performs ongoing evaluations of patient's compliance and response to therapy, and adjusts plan of care accordingly.
- Initiates, provides, or delegates patient/family teaching at a level of expertise and documents appropriately, noting the recipients’ participation and response including pre-procedure calling and education.
- Acts as a patient/family advocate and intervenes if necessary to ensure that patient care is not compromised.
- Participates in planning, scheduling, and coordinating patient care.
Requirements
- Registered Nurse (RN) - Board of Nursing - An active license in the state of practice is required upon hire.
- 1-3 years experience working in a clinical or hospital environment, preferred.
Qualifications
- The individual must be able to work the hours specified.
Skills
- Knowledge of exercise principles and rehabilitation plans.
- Ability to assess and evaluate patient care needs.
- Effective communication and teaching skills.
- Ability to act as a patient advocate.
- Organizational and scheduling skills.
