Utilizing the nursing process, you will provide high-quality direct and indirect patient-centered care, coordinating with the interdisciplinary team. You will communicate patient clinical conditions, respond to changes in condition or treatment, and create a safe, structured, and therapeutic milieu with age-specific behavioral management practices. This role reports to the Clinical Coordinator and promotes a caring, compassionate environment through core values of teamwork, respect, and communication.

Responsibilities

• Performs comprehensive assessments and re-assessments of the patient’s condition and care needs, including psychosocial, physical, educational, and cultural aspects.
• Assesses and reassesses pain, utilizing appropriate pain management tools and techniques. Educates patients and families regarding pain management, with documentation reflecting assessments, interventions, and effectiveness.
• Develops and revises individualized treatment plans based on patient assessment and response to treatment. Evaluates the overall plan daily for effectiveness and involves the patient and family in goal formulation.
• Follows the five medication rights to reduce potential for medication errors. Demonstrates knowledge of medications, including psychotropic drugs, and their correct administration based on patient age and clinical condition.
• Performs all aspects of patient care in an environment that optimizes patient safety and promotes positive outcomes.
• Documents the nursing process and responses to care in a timely manner using appropriate tools.
• Demonstrates flexibility, organization, and critical thinking under stressful situations, with sound judgment in priority setting and delegation.
• Formulates and implements a teaching plan based on identified learning needs, evaluating effectiveness and including family as appropriate.
• Practices autonomously, consistent with evidence-based standards, and pursues personal and professional growth and development. Serves as a professional role model and mentor.
• Maintains a safe and therapeutic environment, demonstrating clinical expertise, perceptiveness, empathy, and interpersonal warmth.
• Displays and promotes sensitivity to the privacy needs of psychiatric patients.
• Effectively uses psychological and physical management techniques to manage violent or potentially violent situations, consistent with PMT training.
• Participates in Early Response codes, Dr. Strong codes, and PMT refresher trainings.
• Demonstrates the ability to set limits with patients and maintain appropriate professional and personal boundaries.
• Collaborates and communicates with all members of the treatment team to meet patient treatment needs.
• Provides psychotherapeutic treatment to patients, including documentation of individual psychotherapeutic interventions and nurse-led educational and psychotherapeutic groups.

Qualifications

• Graduate of an accredited school of Nursing with a current Registered Nurse license in Connecticut.
• Certification in Cardio Pulmonary Resuscitation (CPR) upon hire, with maintained clinical competency during employment.
• Training in PMT and de-escalation techniques, with certification within 6 months of hire.
• Ability to communicate effectively in English, both verbally and in writing.
• Basic computer skills.
• Capability to take swift and efficient action, including patient restraint, during emergency codes.
