Registered Nurse (RN)

Role Overview

The Registered Nurse in Home Health provides and oversees nursing care for patients in their homes, ensuring adherence to physician orders, laws, and agency policies. They coordinate care plans with healthcare professionals, patients, families, and community resources to maintain continuity of care.

Key Responsibilities

• Develops, implements, and revises individualized care plans in collaboration with physicians and team members.
• Supervises home health aides and licensed practical nurses, providing instructions and ensuring regulatory compliance.
• Performs comprehensive assessments, documents visits accurately, and completes OASIS data sets.
• Ensures quality care delivery, communicates effectively with patients, families, and healthcare providers, and maintains timely documentation.
• Participates in quality improvement, survey readiness, and performance enhancement activities.
• Supports team training and on-call duties as needed.

Qualifications & Skills

• Current, unrestricted RN license in the practice state.
• At least one year of Home Health experience.
• Valid driver's license, vehicle insurance, and reliable transportation.
• CPR certification and ability to serve diverse patient populations.
• Preferred: independent work skills, strong communication, organization, and writing abilities.
