  • POSITION HIGHLIGHTS
    • Holistic Patient Care & Assessment
    • Perform comprehensive clinical assessments, administer medications/treatments, and develop individualized care plans to optimize patient recovery and minimize re-hospitalizations.
    • Team Leadership & Delegation
    • Supervise and delegate care to LPNs and CNAs in accordance with state practice acts, fostering a supportive and safe clinical environment.
    • Interdisciplinary Collaboration
    • Work closely with attending physicians, interdisciplinary teams, patients, and families to manage care plans and ensure safe discharge planning.
    • Mentorship & Onboarding
    • Assist with the orientation, training, and clinical mentorship of newly hired nursing staff to support team growth.
    • Modern Documentation
    • Maintain accurate, real-time clinical documentation utilizing PointClickCare (PCC) EHR software.
    • Regulatory Compliance & Safety
    • Adhere to HIPAA standards, infection control protocols, and facility safety guidelines to maintain high-quality care.

    Qualifications

    • Education: Graduate of an accredited School of Nursing (Associate Degree in Nursing - ADN, or Bachelor of Science in Nursing - BSN).
    • Licensure: Active, current, and unencumbered Registered Nurse (RN) license issued by the State Board of Nursing.
    • Certifications: Current BLS / CPR certification (Basic Life Support for Healthcare Providers).
    • Clinical Experience: Experience in long-term care, post-acute care, rehabilitative, or geriatric nursing is preferred, but strong clinical foundational skills are welcome.
    • Language & Communication: Proficient in reading, writing, and speaking English to ensure clear clinical documentation and patient safety.
    • Health Requirements: Must fulfill required health screenings, including Tuberculin Skin Test (TST) verification as mandated by state guidelines.
