About the role
The Registered Nurse (RN) reports directly to the Director of Nursing and is responsible for delivering, coordinating, and evaluating resident care in alignment with physician orders, individualized service plans, and regulatory standards.
Responsibilities
- Provide services in accordance with nurse practice act and state/federal nursing center regulations.
- Work with the Director of Nursing to provide direction and oversight to the care staff regarding the resident’s needs.
- Review and revise assessments and care plans at admission, periodically per regulation, and at change in resident physical or psychosocial condition.
- Review and transcribe physician's orders and provides medical treatments as ordered by physician along with assisting physician with resident examination, as needed.
- Initiate a resident education plan according to the individualized needs of the resident, as prescribed by Physician and/or community policy including resident and family instruction.
- Observe residents and communicate condition changes to physicians, health care proxies, and supervisors and provides timely interventions as instructed.
- Administer and document medications in accordance with the five rights: right medication, right route, right resident, right time, and right dose.
- Supervise and monitor medications for residents who self-administer if applicable.
- Document all pertinent/relevant observations, red flags, incidents, interventions, communication, clinical procedures, and daily medication assistance/care throughout the shift as indicated.
- Absist and monitor residents with meeting their nutritional/hydration needs and documents on the nursing record appropriately.
- Absist residents with activities of daily living as needed, including bathing/showering to schedule, oral care, toileting, dressing, etc.
- Absist residents in scheduling medical, dental and laboratory appointments.
- Utilize proactive intervention use, monitoring, and safety in the rooms and rounds.
- Answer residents call light promptly.
- Absist residents with transfers and ambulation as directed, using proper body mechanics.
- Take temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess the condition of resident as requested.
- Maintain restraint free environment at all times.
- Support fall prevention programs for all residents in need.
- Respond to life saving situations based upon nursing standards, policies, procedures, and protocol.
- Respond as appropriate to personal calls from other health center units in the community.
- Review resident service plans for assisted living and memory care.
- Encourage residents to participate in scheduled facility activities providing assistance in getting to activities as needed.
- Assist in post-mortem care as instructed.
- Promote the physical, personal, and emotional well-being of each resident and strive to maintain the ultimate goal, which is to return the resident to their maximum level of self-care and independence, when possible.
- Understand the residents’ Advance Directives and follows their wishes.
- Ensure effective communication is being utilized by community staff with the residents’ families concerning residents’ care, encompassing their physical, personal, and emotional wellbeing.
- Work well with others and take direction from management, as well as taking initiative with the capability to engage in conflict intervention and resolution for residents, families, employees, and others as needed.
Qualifications
- Graduation from an accredited nursing school and a current state license for LPN or RN is required at all times.
- 1+ years of nursing experience preferred but new graduates are welcome.
- CPR or BLS certification preferred.
- Demonstrated proficiency in common digital tools and applications, including Microsoft Office programs (Outlook, Excel, Word), web-based platforms, electronic health record systems, and any HR-related systems as applicable.
