Schedule

• Monday-Friday 8:00AM-5:00PM
• On-call 5 days per month (3 weekdays, 2 weekend days)

Territories covered: Birmingham, Franklin, Southfield, Bloomfield Hills, Keego Harbor, Pontiac, Rochester, Auburn Hills, Waterford, Clarkston, Davisburg, Lake Orion, Oakland Township, Leonard, Oxford, Holly, Ortonville, Brighton, New Hudson, South Lyon, West Bloomfield, Farmington Hills, Highland, Novi, Milford, Commerce Township, White Lake, Walled Lake, Wixom, Huntington Woods, Northwest Detroit, Oak Park, Redford.

About the role

As a Registered Nurse (RN) the work you do every day makes a difference in the lives of our patients. As an RN you will have the opportunity to provide one on one patient care and work at the top of your license. Responsibilities

• Provide comprehensive skilled care as ordered by the attending physician within specified geographical territory.
• Work autonomously, efficiently managing your time.
• Observe and monitor patient conditions and perform OASIS assessments (training provided if needed) to develop an individualized care plan and adjust as needs change.
• Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients.
• Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Educate the patient and family on disease processes using teach-back methods to ensure understanding.
• Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
• Understand and follow agency policies, procedures, rules, and regulations.
• Communicate changes in schedule/availability to schedulers or supervisors.
• Other duties as assigned.

Requirements

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting.
• Home care experience preferred.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands of the role.
