About the Role
Working for the Facility Resource Pool, you will provide clinical direction for all aspects of patient care in an acute inpatient setting, fostering patient/family-centered care. This role requires a willingness to float to various nursing units at the assigned Prisma Health facility and may include weekend, holiday, and on-call work.
Responsibilities
- Collects data and completes nursing assessments using appropriate area and/or age-specific standards, policies, and tools. Identifies subtle or actual changes in patient status and response to intervention.
- Develops an individual plan of care in collaboration with the patient, family, physician, and interdisciplinary team. Updates the plan based on changes in patient condition.
- Identifies realistic and measurable goals and outcomes for discharge, ensuring continuity of care and smooth transitions.
- Provides direct patient care consistent with the nursing plan, delegating tasks as appropriate to healthcare team members.
- Monitors quality of care, ensuring patient safety and adherence to protocols, cost containment, and quality improvement principles.
- Evaluates the effectiveness of the care plan, revising as needed and communicating changes to the healthcare team.
- Assesses and addresses patient/family learning needs, providing education using clear language and evaluating understanding.
- Communicates patient/family educational needs across the healthcare continuum.
- Evaluates own nursing practice against professional standards, maintaining competency through education and licensure.
- Ensures documentation and communication focus on pertinent information to support patient care needs.
- Serves as the patient’s advocate and coordinator of care, collaborating with physicians and the healthcare team.
- Adheres to organizational policies, including licensure renewal, training, employee health screenings, and regulatory requirements (TJC, DHEC, CMS).
- Performs other duties as assigned.
Requirements
- Associate degree in Nursing (Bachelor’s degree preferred).
- Two (2) years of experience as a registered nurse.
- Licensed to practice as a Registered Nurse in South Carolina.
- Knowledge of regulatory agency requirements (TJC, DHEC, CMS).
- Ability to provide care within the scope of the South Carolina Nurse Practice Act.
This is a non-management role reporting to a supervisor, manager, director, or executive.
