Registered Nurse (RN) - Acute Care, Richland Facility Resource Pool, FT, Days

About the Role

Working for the Facility Resource Pool, you will provide clinical direction for all aspects of patient care in an acute inpatient setting, fostering patient/family-centered care. This role requires willingness to float to various nursing units at the assigned Prisma Health facility and may include weekend, holiday, and on-call work.

Responsibilities

• Collects data and completes nursing assessment utilizing appropriate area and/or age-specific standards/policies and tools. Assesses and recognizes subtle or actual changes in patients' status and response to intervention.
• Identifies nursing priorities for patient plan of care based on nursing assessment and validates these priorities with healthcare team members.
• Develops an individual plan of care in collaboration with the patient, family, physician, and interdisciplinary healthcare team. Updates the plan based on changes in patient condition.
• Identifies realistic and measurable goals and outcomes to be attained prior to discharge and validates these with the patient and/or family.
• Demonstrates respect for the patient's/family's cultural, spiritual, and belief system in planning for care.
• Plans for discharge to ensure continuity of care and a smooth transition post-discharge, including appropriate referrals.
• Provides direct care consistent with the established nursing plan and performs specific tasks based on organizational, departmental, and unit policies.
• Delegates nursing tasks appropriately to members of the healthcare team and ensures completion of assigned tasks.
• Monitors quality of care delivered and ensures appropriate surveillance of assigned patients.
• Coordinates and organizes tasks, incorporating cost containment principles into nursing care.
• Assumes responsibility for patient safety by maintaining a safe environment and adhering to safety procedures and protocols.
• Utilizes appropriate technology and performance indicators to monitor quality and effectiveness of nursing care.
• Evaluates the plan of care for effectiveness, revises as needed, and communicates changes to the healthcare team.
• Identifies and assesses patient/family learning needs and provides education using clear, familiar language. Evaluates and encourages patient/family involvement in the teaching plan.
• Communicates the patient/family's need for education to others in the healthcare continuum.
• Evaluates own nursing practice in relation to professional standards, policies, and regulations. Maintains professional licensure and completes mandatory education.
• Ensures documentation and communication focus on pertinent information to support patient/family care needs.
• Serves as the patient's advocate and coordinator of care, collaborating with physicians and other healthcare team members.
• Adheres to all compliance and policy/procedures of the organization, including licensure renewal, training, health screenings, time and attendance, dress code, patient confidentiality, infection control, and medication administration.
• Demonstrates knowledge of regulatory agency requirements (TJC, DHEC, CMS) and provides care within the scope of the South Carolina Nurse Practice Act.

Requirements

• Associate degree in Nursing (Bachelor's degree preferred).
• Two (2) years of experience as a registered nurse.
• Licensed to practice as a Registered Nurse in South Carolina.
