Responsibilities

• Provide direct patient care to our residents, including administering medications, performing vital signs, and monitoring patients' conditions
• Develop and implement individualized care plans, in collaboration with our interdisciplinary team
• Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
• Participate in quality improvement initiatives and contribute to our facility's commitment to excellence in patient care
• Demonstrate a commitment to our facility's mission and values, and uphold our standards of excellence in patient care

Qualifications

We are seeking a compassionate and skilled Registered Nurse (RN) to join our dynamic team at Woburn Rehabilitation and Nursing Center. Candidates must hold a current RN license in Massachusetts and possess a strong background in patient care and teamwork.
