About the Role

As a Clinical Team Manager with Elara Caring, you shape the future of healthcare. Responsibilities

• Manage a team of registered nurses and other healthcare professionals
• Ensure compliance with Medicare/Medicaid home care benefits, policies, and procedures
• Implement QAPI programs to improve clinical outcomes
• Develop and maintain positive relationships with clients, families, and other healthcare providers
• Train and mentor team members to enhance their skills and knowledge
• Oversee daily operations, ensuring quality patient care
• Collaborate with other departments to provide seamless care

Requirements

• High School Diploma or GED
• Bachelor’s degree preferred
• 3 years of active experience as a RN, preferably with 1 year in the home care industry
• Knowledge of Medicare/Medicaid home care benefits, policies, and procedures
• Valid driver’s license and insurance
• Reliable transportation to perform job tasks
