Responsibilities

• Clinical Execution & Physician Partnership: Accurately execute complex medical orders and treatment strategies while partnering with physicians to deliver expert, patient-centered care.
• Interdisciplinary Coordination: Serve as the central communication hub for the interdisciplinary team to ensure seamless collaboration and holistic, well-integrated care journeys.
• Patient Advocacy & Health Monitoring: Proactively track health trends and document progress to advocate for optimal outcomes and provide the medical team with timely insights for life-changing interventions.
• Discharge & Transition Leadership: Lead comprehensive transition planning, educate patients and families, and provide tools for confident recovery at home.

Requirements

• Licensure / Certification / Registration:
  • Advanced Life Support obtained within 6 months (180 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted).
  • BLS Provider obtained within 1 month (30 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted).
  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition “Compact State” obtained prior to hire date or job transfer date.
• Education: Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.
