Registered Nurse (PRN)

About the Role

We are seeking a dedicated Registered Nurse (RN) to provide compassionate home health care services in a supportive and team-oriented environment.

Responsibilities

• Perform client assessments and develop individualized care plans based on agency standards.
• Manage case coordination, supervision, and implementation of nursing and supportive services.
• Respond to referrals, inquiries, and initiate appropriate in-home care actions independently.
• Ensure compliance with agency policies, procedures, and professional standards.
• Perform additional duties as assigned by the Director of Clinical Services.

Qualifications and Skills

• Current RN license in Colorado.
• Minimum of two years of nursing experience in long-term care, dementia care, or acute care; home care experience preferred.
• Graduate of an accredited nursing program; Baccalaureate degree preferred.
• CPR certification required.
