Responsibilities

• Conducting client assessments and developing individualized care plans
• Providing case management, supervision, and coordination of nursing services
• Responding to client referrals and inquiries effectively
• Performing duties in accordance with agency policies, procedures, and professional standards
• Performing other responsibilities as assigned by leadership

Requirements

• Graduate of an accredited nursing program; Baccalaureate preferred
• Current RN license in Colorado
• Minimum of 2 years nursing experience, with a preference for home care, long-term care, dementia care, or acute care
• CPR certified
