Legacy Hospice is seeking a Per Diem Hospice RN Case Manager to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community. As a valued member of our interdisciplinary care team, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey, ensuring every patient receives the best care possible—care grounded in compassion, clinical excellence, and connection.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Partner closely with the Clinical Director to assess eligibility and suitability of clients for specialized hospice services.
• Provide direct nursing care, ensuring patients’ comfort and quality of life remain paramount.
• Deliver top‑notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
• Educate and advocate for patients and families, empowering them to participate in creating personalized care plans aligned with their goals.
• Share call duties, demonstrating commitment to being there when it matters most.

Qualifications

• Hold a current, unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical‑surgical/acute care setting; hospice experience a plus.
• Computer proficient in typing and various programs, with background in EMR systems.
• Possess and maintain current CPR certification if required by state.
• Current driver’s license, vehicle insurance, and access to dependable transportation.

Pay

$39.00 – $43.00 per hour (average pay range). Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
