Schedule

Per Diem Flex Schedule – Monday through Friday

Pay

Attractive pay $35.00 to $45.00 per hour for orientation, then conversion to a per‑visit pay method.

Responsibilities

• Coordinate and oversee all aspects of patients' home care.
• Collaborate with patient care teams to develop and implement care plans.
• Educate families on patient condition, needs, and care.
• Perform head‑to‑toe physical assessments.
• Check vital signs and manage medications.
• Give injections, IV therapy, or tube feedings.
• Administer and maintain catheters.
• Complete documentation timely and accurately.
• Supervise LPNs and HHAs.
• Regularly communicate patient progress to the clinical manager, physician, and care team.
• Other duties as assigned.

Qualifications

• One year of clinical experience as a registered nurse (RN). If less than one year, candidate must be approved by VP Clinical.
• Current RN license for the state(s) of assignment.
• Minimum six months home health experience.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and liability insurance.
• Ability to assess patient status and identify age‑specific needs.
• Familiarity with general computer use (user names, passwords, internet, email, desktop navigation, online HR services, and training programs).
• If less than six months clinical experience as an RN, candidate must participate in the RN Intern program.
