Registered Nurse Primary Care Clinic

About the role

Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data. Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.

Responsibilities

• Demonstrate competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocate for and protect the health, safety, and rights of each patient.
• Provide patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourage patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Deliver relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegate tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Coordinate and implement group visits for identified high-risk patient populations.
• Aid in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contribute to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
• Other duties as assigned.

Qualifications

• Associate's of Nursing (Required)
• Bachelor's of Nursing (Preferred)
• Registered Nurse (RN) license (Required)
• Basic Life Support - CPR Cert (BLS) (Required)

Full time, Monday - Friday 8:30am-5pm

Pay

$31.60 - $50.56
