Registered Nurse - PMC - Special Care Nursery - F/T Nights
About the role
As a Registered Nurse at Hackensack Meridian Health, you will function as a caregiver for a specific patient population to achieve desired outcomes. This role focuses on expanding knowledge and skills, providing effective direct care as part of the interdisciplinary team for a variety of complex patients. You will direct and oversee care provided by other caregivers through delegation, validation, evaluation, and coordination, while effectively coordinating patient care and unit activities among nursing peers, physicians, and support services.
Responsibilities
- Assessment
- Collect comprehensive data pertinent to the patient's health or situation.
- Perform age- and disease-specific assessments independently in a systematic manner, focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic changes in patient status.
- Identify patient/family education needs based on age, disease, and culturally specific learning differences.
- Plan
- Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulate age- and disease-specific, patient/family-centered goals and outcomes based on assessment data.
- Develop and coordinate a patient/family-focused plan of care that is prioritized and encompasses the continuum of care.
- Implementation
- Communicate, coordinate, and implement the plan of care.
- Provide ongoing communication with the patient/family, nursing colleagues, and other members of the healthcare team regarding the patient’s status, rendered care, and treatment plans.
- Implement age- and disease-specific care in an organized and timely manner.
- Provide or coordinate patient/family education as needed and ensure understanding.
- Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
- Provide nursing care and treatments that reflect the patient’s needs, advocate appropriately, and demonstrate evidence-based practice and compliance with standards.
- Perform continuous Point-of-Care electronic documentation of each patient’s assessments, plan-of-care, interventions, and evaluation of rendered care.
- Administer medication utilizing bar-scanning and documentation technologies to ensure safe delivery.
- Coordinate multi-disciplinary discharge of patients to ensure safe transitioning and continuum of care.
- Evaluation
- Evaluate progress toward attainment of outcomes.
- Utilize critical thinking skills based on nursing knowledge to achieve desired patient outcomes.
- Initiate interventions related to changes in patient condition and unexpected responses to care.
- Additional Responsibilities
- Identify ethical issues in the practice setting and bring them to the attention of other team members.
- Recognize and respond to opportunities to enhance patient satisfaction.
- Acquire and maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
- Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
- Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand a minimum of 6 hours a day.
- Adhere to HMH’s Organizational and Managerial competencies and standards of behavior.
- Perform other duties and/or projects as assigned.
Qualifications
- Education, Knowledge, Skills, and Abilities Required:
- BSN in progress and/or willingness to acquire within three (3) years of hire (applies to team members hired on or after November 2019).
- Six (6) or more months of relevant nursing experience OR active participation in Hackensack Meridian Health's Nurse Residency Program.
- Ability to learn quickly and adapt to changing patient needs.
- Strong sense of accountability for improving the lives of patients and their caregivers.

- Passion for delivering the highest quality of care based on a strong sense of patient focus.
- Education, Knowledge, Skills, and Abilities Preferred:
- National Certification in area of specialty (preferred when eligible).
- Licenses and Certifications Required:
- NJ State Professional Registered Nurse License.
- AHA Basic Health Care Life Support HCP Certification within 60 days of entering the position.
