Registered Nurse - Physical Rehabilitation

Role Overview

The position involves leading personalized rehabilitation care plans, executing physician and nursing orders, monitoring patient progress, and facilitating discharge planning to ensure safe and effective recovery in an acute rehab setting.

Responsibilities

• Lead implementation of individualized care plans to promote patient independence.
• Accurately execute clinical orders to support long-term recovery.
• Monitor and document patient progress, communicating vital updates to the care team.
• Capture comprehensive discharge planning with multidisciplinary teams for smooth patient transitions.

Qualifications & Skills

• Current RN licensure from the Indiana Board of Nursing.
• BLS certification within one month of hire (American Heart Association or Red Cross accepted).
• Prior nursing experience preferred.
