Registered Nurse (per-diem)

Key Responsibilities

• Provide nursing assessments for each participant upon admission, change in status and discharge.
• Cookordination participant's care with all relevant parties (family, guardian, physicians, nursing facilities, and residential providers) and maintain appropriate documentation of such care.
• Serve as a brand ambassador for Vitra reflecting our vision, mission, and values.
• Show a genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members.

What We Are Looking For

• RN Massachusetts license required
• Previous experience in an Adult Day Health setting or comparable work experience
• 1-2 years of direct service experience working with the elderly, chronically disabled, individuals with memory loss and/or Dementia
• Excellent knowledge of clinical standards and regulations
• Fluency in both English and Cape Verdean Creole or secondary language is preferred
• Strong time management and problem-solving skills with the ability to prioritize
• Outstanding written and verbal communication skills, with close attention to detail
• Strong organizational skills with the ability to multitask
• Able to work independently and in a team environment

Physical and Environmental Demands

• Works in a clean well-lit environment with fluctuating temperatures in close proximity to others.
• Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.
• Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds;
• Requires frequent sitting more than 75% of the workday;
• Requires the ability to negotiate stairs;
• Requires visual acuity and manual dexterity to operate equipment.
