The PECAN Program is a comprehensive, multidisciplinary nutrition program within the Heart Center that provides individualized, highly specialized support for pediatric patients with congenital heart disease.
About the role
The team is seeking a full-time nurse to support programmatic growth and care coordination for this special population.
Responsibilities
- Provide team–based care to patients with known congenital or acquired heart disease and arrhythmias, requiring cardiology and nutritional team interventions for growth and nutritional evaluation.
- Support patient care in both Inpatient (Boston) and Outpatient Cardiology.
- Attend clinic sessions to facilitate patient flow, complete nursing assessment, and provide patient and family education.
- Ensure care coordination and timely follow-up by communicating with families, primary care providers, other specialists, and community agencies.
- Provide discharge planning including patient and family education and coordination with the inpatient unit for transition to home care.
- Triage phone calls from parents and outside providers as well as assist with provider messages and care coordination.
- Support, then transition to oversight of Outpatient “Cares for Kids” visits (Mass Health initiative).
- Support, then transition to oversight of Remote Patient Monitoring within PECAN.
Requirements
- BSN degree.
- At least 2 years of ambulatory or inpatient pediatric nursing experience preferred, with preference for pediatric cardiac care or GI experience.
- Strong knowledge of NG tube placement and care.
- Strong written and verbal communication, organizational, and collaboration skills.
- Preference for EPIC EMR and Prior Authorization experience.
Qualifications
- MA RN License.
- BLS required, PALS required within 1 year.
Schedule
40 hours per week, working at Boston and Waltham satellite locations.
Pay
The posted pay range is Boston Children’s reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
