Registered Nurse – Patient-Focused Home Health Care

About the role

New Dimensions Home Health Care is expanding its services and is seeking a Registered Nurse (RN) to join their home health team in the Fergus Falls area. This is a flexible, full-time position with the option to build your own schedule.

Responsibilities

• Perform INR draws and medication management support
• Serve as a liaison between clients and physicians
• Complete patient assessments and accurate documentation
• Supervise Personal Care Assistants (PCAs)
• Support coordinated, patient-centered care plans
• Ensure continuity of care and communication across providers

Requirements

• Active Registered Nurse (RN) license required
• Strong assessment and clinical judgment skills
• Highly organized and detail-oriented
• Ability to work independently in the field
• Strong communication and interpersonal skills
• Career-focused and patient-focused approach

Qualifications

• Experience in home health care preferred
• Previous experience with medication management and patient assessments
• Knowledge of personal care assistance protocols
