About the role

New Dimensions Home Health Care is expanding its services and is seeking a Registered Nurse (RN) to join their home health team in the Fergus Falls area. This is a flexible, independent opportunity to provide high-quality, person-centered care.

Responsibilities

• Perform INR draws and medication management support
• Serve as a liaison between clients and physicians
• Complete patient assessments and accurate documentation
• Supervise Personal Care Assistants (PCAs)
• Support coordinated, patient-centered care plans
• Ensure continuity of care and communication across providers

Requirements

• Active Registered Nurse (RN) license required
• Strong assessment and clinical judgment skills
• Highly organized and detail-oriented
• Ability to work independently in the field
• Strong communication and interpersonal skills
• Career-focused, patient-focused approach to care

Qualifications

• Experience in home health care preferred
• Knowledge of home health care regulations and standards
• Experience with personal care assistance supervision
