About the role

We’re searching for a compassionate, dedicated, and dependable Part-Time Registered Nurse (RN) who is committed to making a difference in the lives of others. This individual will perform in-home client assessments, create individualized Service Plans/Care Plans, orient and monitor aides providing services, and conduct supervisory visits in the client’s residence.

Responsibilities

  • Oversee client services and ensure client needs are met.
  • Handle annual evaluations, competencies, state regulations, and accreditation (if applicable).
  • Provide and coordinate services in accordance with the Service Plan/Care Plan.
  • Make the initial evaluation visit and regularly reevaluate the client’s needs.
  • Initiate the Service Plan/Care Plan, necessary revisions, and physician orders (if applicable).
  • Initiate appropriate preventive and rehabilitative non-medical procedures.
  • Document client assessments, Care Plans, supervisory visits, caregiver and client education, and training.
  • Inform personnel of changes in the client’s condition and needs.
  • Adhere to established policies and procedures.
  • Observe, record, and document signs and symptoms; report to physicians, administrators, or care coordinators regarding reactions to treatments or changes in the client’s condition.
  • Communicate and counsel clients and families regarding service needs and related problems.
  • Participate in quality assurance reviews and evaluations of the agency’s services.
  • Supervise and teach other team members.
  • Assist with other assigned activities.

Requirements

  • Graduate of a state-approved school of nursing and currently licensed in the state(s) of practice.
  • Minimum of two (2) years of in-home care experience.
  • Minimum of one (1) year of supervisory experience (preferred).
  • Proficient in Microsoft Office, Google Suite, and scheduling software.
  • Maintain current state Registered Nurse (RN) licensure.
  • Valid driver’s license and auto insurance.
  • Excellent organizational, communication, interpersonal, and leadership skills.
  • Positive and professional image.
