Role Overview
Serve as a primary clinical partner providing individualized, in-home nursing care to promote patient recovery, safety, and independence. Conduct comprehensive assessments to develop patient-centered care plans and deliver skilled interventions such as wound care, infusions, medication management, and chronic disease support. Educate and empower patients and caregivers to confidently manage health conditions and daily care. Monitor patient progress, identify early changes, and communicate effectively with interdisciplinary teams to adjust plans and prevent hospitalizations. Ensure care continuity through coordination across providers and complete accurate documentation supporting quality outcomes.
Qualifications & Skills
- Active RN license in applicable state and at least one year of clinical experience.
- Patient-focused, organized, and motivated by delivering high-quality care.
- Valid driver’s license, reliable transportation, and CPR certification or ability to obtain.
- Strong communication skills and ability to collaborate with care teams.
Key Responsibilities
