Registered Nurse that will work in the Continuous Care Services Department in Palliative Care Services
Hours & Shift
Full-time (1.0 FTE) day shift position, Monday through Friday 8 a.m. to 4:30 p.m.
Role Responsibilities
- Works in collaboration with an interdisciplinary team to deliver palliative care for patients and families who are dealing with serious chronic illnesses
- Helps educate patients and families with a focus on improving symptom management as well as increasing knowledge of medical options and how they impact quality of life
- Utilizes the professional nursing process to provide individualized nursing care to patients across the lifespan
- Coordinates patient care, screenings, education, triage, and various other tasks within the RN scope of practice
- Develops and maintains care plans with patients in collaboration with the patient’s care team
- Accurately documents all nursing care and related services in the patient health record
- Provides for safe patient transitions and handoffs through effective communication with patients, family and care providers
- Manages and prioritizes multiple assignments and tasks while setting priorities and adapting to changing conditions
- Communicates clearly, concisely, and logically both verbally and in writing
- Maintains confidentiality and builds trusting relationships with patients and colleagues
Qualifications
- Associates or Bachelor Degree in Nursing
- Current and valid Registered Nurse License from the State of Wisconsin
- Certified Basic Life Support (BLS)
- One to two years of experience in a clinic or hospital setting
- Palliative Care experience preferred
- Knowledge of motivational interviewing
- Chronic Care Management and Hospice background also preferred
- Prior experience using the EPIC medical record system is preferred but not required
- Prior experience with the Microsoft Office Suite software programs is beneficial
