About the role

Partner with patients and families to design, implement, and monitor tailored care plans that anticipate pain, alleviate complex symptoms, and maximize daily functional capacity and comfort. Serve as the primary coordinator across all care disciplines, ensuring a seamless, holistic approach that honors the patient's clinical needs while embracing their family's emotional, psychological, spiritual, and bereavement journeys. Accurately note and carry out physician and nursing orders with the highest standard of clinical precision. Support vital consultations across our hospitals, including Via Christi St. Francis, St. Joseph, and St. Teresa campuses.

Requirements

  • BLS Provider obtained within 1 month (30 days) of hire date or job transfer date (American Heart Association or American Red Cross accepted)
  • Licensed Registered Nurse credentialed from the Kansas Board of Nursing obtained prior to hire date or job transfer date
  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire

Preferred qualifications

  • Certified Registered Nurse specializing in Hospice/Palliative Care credentialed from the Hospice & Palliative Credentialing Center (HPCC)
  • Experience in hospice or palliative care
  • Availability to work during standard office hours of Monday through Friday, 8:00 AM – 4:30 PM
