Job Details
We are hiring a Registered Nurse for an Adult Medicine department in a busy outpatient healthcare setting. This role is ideal for an RN, Primary Care RN, Outpatient RN, Ambulatory RN, Clinic RN, Triage RN, or Adult Medicine RN who enjoys patient education, care coordination, chronic care management, triage, and working as part of a collaborative clinical team.
Responsibilities
- Provide professional nursing care in an Adult Medicine / Primary Care setting
- Triage patients by phone and in person to prioritize sick visits, home care instructions, and provider follow-up
- Administer medications and treatments according to provider orders and nursing standards
- Complete nurse visits to support chronic care management and provider efficiency
- Perform point of care testing and support quality control documentation
- Communicate referral, hospitalization, diagnostic imaging, and lab result updates to the clinical team
- Support infectious disease follow-up and required public health reporting
- Prepare exam rooms, maintain equipment, and support safe clinical operations
- Assist with clinical procedures as appropriate
- Support infection control, equipment maintenance, and quality logs
- Assist with medication, vaccine, and clinical supply inventory
- Provide patient education, health coaching, and self-management support
- Help identify gaps in care using population health and data reporting tools
- Care for lab, diagnostic, follow-up, home care, and referral needs
- Manage moderately complex patient, pharmacy, and external provider calls
- Support prior authorizations as needed
- Communicate social determinants of health concerns to providers and care team members
- Maintain CPR, HIPAA, compliance, waived testing, infection control, and other required training
Qualifications
- Active Connecticut Registered Nurse license
- Associate’s degree in Nursing required
- CPR certification at the healthcare provider level, or ability to obtain within 90 days
- Strong communication, documentation, and clinical judgment skills
- Comfort entering information into an electronic health record
- Ability to work independently and in a team-based environment
- Ability to manage competing priorities in a fast-paced outpatient setting
- Commitment to confidentiality, professionalism, and patient-centered care
- Preferred: BSN, Adult Medicine, Primary Care, Internal Medicine, Family Medicine, Ambulatory Care, Outpatient Nursing, or FQHC experience, Spanish-speaking ability, Experience with PCMH, population health, chronic care management, triage, or care coordination
