Registered Nurse or LPN - Support Center - PRN
Position Summary
An employee in this position will manage referrals to The Organization, coordinating the process through the admission of the patient while managing referral databases. This employee will also be performing telehealth triage functions including but not limited to receiving calls; providing phone support; providing interventions utilizing BCN policies, procedures, and protocols; dispatching on-call nurses as needed, completing documentation utilizing the EMR, point of service and making appropriate notifications to primary team regarding any changes in LOC.
Position Qualifications
- Registered Nurse or Licensed Practical Nurse with a minimum of three years of Hospice, home health, Palliative, Primary Care or critical care/ED experience preferred.
- Telehealth experience also preferred.
- Ideal candidate will demonstrate strong communication skills and customer service mindset.
- Ability to exercise judgment and discretion in confidential matters.
- Has excellent assessment, nursing judgment, and communication skills.
- Demonstrated ability to respond to the needs of patients, families, and referral sources in varied settings.
- Able to cope with heightened family emotional stress, and tolerant of individual lifestyles.
- Able to partner with multiple clinical teams to ensure excellence in customer service.
- Must have strong working knowledge of phone and computer equipment.
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and has the ability to work with an interdisciplinary team.
