Registered Nurse or LPN - Memory Care - Full time Days - Helen Porter Rehabilitation & Skilled Nursing
Helen Porter Rehabilitation & Nursing, part of the University of Vermont Health Network and Porter Medical Center, is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care Unit on the day shift in Middlebury, Vermont.

Our Memory Care Unit provides specialized care for residents living with Alzheimer's disease and other forms of dementia in a safe, supportive, and nurturing environment focused on dignity, purpose, and quality of life.

Responsibilities
- Provide skilled nursing care to residents with Alzheimer's disease and other memory-related conditions
- Assess, monitor, and document resident health status and changes in condition
- Administer medications and treatments according to physician orders
- Promote resident safety, comfort, and well-being
- Collaborate with nursing assistants, providers, rehabilitation staff, and other members of the care team
- Communicate effectively with families and caregivers
- Support person-centered care plans that honor each resident's preferences and needs
- Maintain accurate clinical documentation and ensure compliance with regulatory standards
- Contribute to a positive, compassionate, and team-focused culture
Requirements
RN Candidates
Current Vermont Registered Nurse (RN) license or eligibility for Vermont licensure
LPN Candidates
Current Vermont Licensed Practical Nurse (LPN) license or eligibility for Vermont licensure
Qualifications
- Experience in memory care, dementia care, long-term care, skilled nursing, assisted living, rehabilitation, hospital, or acute care settings
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Commitment to delivering compassionate, resident-centered care
- Ability to work effectively as part of a collaborative team

Full-Time, 36 hours per week
Day Shift: 6:45 AM - 3:15 PM
