Registered Nurse or LPN - Long Term Care - Full time night - Helen Porter Rehabilitation & Skilled Nursing

About the role

Helen Porter Rehabilitation & Nursing, part of the University of Vermont Health Network – Porter Medical Center, is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our overnight nursing team on a long-term care assisted living unit.

Responsibilities

• Provide skilled nursing care to long-term care and assisted living residents
• Assess resident conditions and communicate changes appropriately
• Administer medications and treatments according to physician orders
• Monitor resident health, safety, comfort, and overall well-being
• Complete accurate and timely documentation in the electronic medical record
• Collaborate with providers, therapists, nursing assistants, and interdisciplinary team members
• Serve as an advocate for resident-centered care and quality outcomes

Qualifications

• RN Candidates: Current Vermont Registered Nurse (RN) license in good standing, long-term care, skilled nursing, rehabilitation, or geriatric nursing experience preferred
• LPN Candidates: Current Vermont Licensed Practical Nurse (LPN) license in good standing, long-term care, assisted living, or skilled nursing experience preferred
• Strong clinical assessment and communication skills
