Job Responsibilities

Maintains knowledge of CLTC programs and related systems including Phoenix information system and assists other CLTC offices with essential job functions when scheduling permits.

Conducts pre-admission screening for persons seeking Medicaid sponsored nursing home placement and for persons seeking admission to various Medicaid waivers and programs such Community Choices, HIV-AIDS, HASCI, Children's Personal Care, TEFRA, and PASRR.

Participates in and/or conducts training on PASRR completion and assessment to staff of long-term care facilities and hospitals.

Performs all and ongoing oversight and medical consultation for Ventilator Waiver and Children’s Personal Care program.

Travel to location of assigned cases is necessary for this position.

Provides Education and Referral.

Performs Assessment Activities.

Performs PASRR activities.

Provides consultation to CLTC case managers.

Promotes good communication.

Provides program related training.

Performs Children's Personal Care Program Activities.

Performs emergency preparedness duties or activities as required.

Performs Other Duties as Assigned.

Minimum And Additional Requirements

• A Diploma in Nursing or an Associate Degree in Nursing and one (1) year of experience as a Registered Nurse.
• Graduation from an accredited school of nursing.

Preferred Qualifications

• Bachelor's degree in Nursing.
• NECESSARY SPECIAL REQUIREMENT: Current licensure as a Registered Nurse by the State Board of Nursing for South Carolina.
