About the role
This role blends clinical care, patient education, and case management. Flexible Monday-Friday 8am-5pm type-of schedule. No Weekends, No Holidays!
Responsibilities
- Train patients and caregivers on PD procedures and self-care
- Manage a caseload of PD patients, including assessments and care planning
- Monitor treatment progress, conduct home visits, and provide on-call support
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
Requirements
- RN license in the state of practice
- Willing to obtain both CT and NY State license (we reimburse)
- Minimum of 12 months of RN experience
- Valid driver’s license, insurance, and reliable transportation
- ADN required; BSN preferred
- Basic computer skills (MS Word, Outlook)
Qualifications
- Dialysis (PD, HHD, chronic, or acute) experience is not required
- Nephrology nursing provides a fresh lens and set of skills to those newly entering the field
Skills
- Excellent communication skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team

Schedule Monday-Friday, 8-hour shifts
Pay
Compensation for the role will depend on a number of factors, including a candidate’s qualifications, skills, competencies and experience.
