About the Role

As a Registered Nurse at Hackensack Meridian Health, you will function as a caregiver for a specific patient population to achieve desired outcomes. This role focuses on expanding knowledge and skills, providing effective direct care as part of the interdisciplinary team for a variety of complex patients. You will direct and oversee care provided by other caregivers through delegation, validation, evaluation, and coordination, while effectively coordinating patient care and unit activities among nursing peers, physicians, and support services.

Responsibilities

  • Assessment, Planning, Implementation, and Evaluation
    • Collect comprehensive data pertinent to the patient's health or situation.
    • Perform age- and disease-specific assessments independently in a systematic manner, focusing on physiologic, psychological, and cognitive status.
    • Recognize clinical and diagnostic changes in patient status.
    • Identify patient/family education needs based on age, disease, and culturally specific learning differences.
    • Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
    • Formulate age- and disease-specific, patient/family-centered goals and outcomes based on assessment data.
    • Develop and coordinate a patient/family-focused plan of care, prioritized and encompassing the continuum of care.
  • Communication and Coordination
    • Provide ongoing communication with patient/family, nursing colleagues, and other members of the healthcare team regarding patient status, care, and treatment plans.
    • Implement age- and disease-specific care in an organized and timely manner.
    • Provide or coordinate patient/family education as needed and ensure understanding.
    • Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
  • Direct Care and Documentation
    • Provide nursing care and treatments reflecting patients' needs, advocate appropriately, and demonstrate evidence-based practice and compliance with standards.
    • Perform continuous Point-of-Care electronic documentation of assessments, plan-of-care, interventions, and evaluation of rendered care.
    • Administer medication utilizing bar-scanning and documentation technologies to ensure safe delivery.
    • Facilitate multi-disciplinary discharge of patients to ensure safe transitioning and continuum of care.
  • Evaluation and Critical Thinking
    • Evaluate progress toward attainment of outcomes.
    • Utilize critical thinking skills to achieve desired patient outcomes.
    • Initiate interventions related to changes in patient condition and unexpected responses to care.
    • Identify ethical issues in practice and bring them to the attention of the team.
    • Recognize and respond to opportunities to enhance patient satisfaction.
  • Professional Development and Compliance
    • Acquire and maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
    • Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
    • Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand a minimum of 6 hours a day.
    • Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
  • Perform other duties and/or projects as assigned.

Requirements

  • A minimum of an Associate in Nursing or diploma in Nursing degree.
  • BSN in progress and/or willingness to acquire within three (3) years of hire.
  • Current and valid Registered Nurse license to work at the assigned HMH location by date of hire.
  • BLS/CPR certification by the American Heart Association required within 60 days of hire.
  • Six (6) or more months of relevant nursing experience OR active participation in Hackensack Meridian Health's Nurse Residency Program.
  • Ability to learn quickly, adapt to changing patient needs, and demonstrate a strong sense of accountability for improving patients' and caregivers' lives.
  • Exceptional focus on teamwork, dedication to ongoing education, and passion for delivering high-quality care based on patient focus.
  • National Certification in area of specialty preferred when eligible.

Pay

The minimum starting rate of pay for this role is $48.18 per hour. The posted rate is a reasonable good-faith estimate and does not reflect the full value of the market-competitive total rewards package. The base compensation may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget or grant funding. Some roles may also be eligible for performance-based incentives, bonuses, commissions, or shift differentials for evening, night, or weekend shifts.
