Schedule

• Full time, day shift
• Monday–Friday, 8:30 AM–5:00 PM (possibility of 10-hour shifts)
• Location: 2363 63rd Street, Woodridge, IL (may travel to clinic in Hinsdale)

Responsibilities

• Assess patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data.
• Develop and implement patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards.
• Demonstrate competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection.
• Advocate for and protect the health, safety, and rights of each patient.
• Provide patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching.
• Encourage patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology.
• Deliver relationship-based care, engaging patients in all aspects of care with respect for cultural differences and diversity.
• Delegate tasks to non-RN personnel in accordance with State Board of Nursing and hospital policy.
• Coordinate and implement group visits for identified high-risk patient populations.
• Aid in the care transition of high-risk patients, including medication reconciliation and coordination of services.
• Contribute to continuous process improvement by actively participating in patient experience and satisfaction initiatives.
• Other duties as assigned.

Requirements

• Associate's of Nursing (Required)
• Bachelor's of Nursing (Preferred)
• Registered Nurse (RN) license (Required)
• Basic Life Support (BLS) CPR certification (Required)
• 1+ years of medical office experience (Preferred)
• 5+ years of critical care experience (Preferred)

Pay

$31.60 – $50.56 per hour
