Job Description

The Nephrology RN delivers high-quality, individualized patient care in accordance with the Lahey Clinic Professional Practice Model, MA Nurse Practice Act, and ANA Standards. This role emphasizes continuity of care, accountability, and collaboration with physicians and interdisciplinary teams. The RN is responsible for assessment, planning, implementation, and evaluation of care; patient education; care coordination; and clinical decision-making across the ambulatory setting. The position includes delegation and supervision of support staff per regulatory standards.

  • Current Massachusetts RN license required

Clinical Judgment & Patient-Centered Care

  • Provide safe, coordinated nursing care through strong organizational and prioritization skills
  • Perform comprehensive patient and family assessments appropriate to the care setting
  • Analyze and interpret clinical data to identify patient needs and guide nursing interventions
  • Develop individualized, culturally responsive plans of care that include:
    • Identified patient problems and expected outcomes
    • Preventive and health-promoting strategies
    • Respect for patients’ religious, spiritual, ethnic, and cultural values
  • Deliver age-appropriate, developmentally informed care
  • Use strong listening skills and clinical judgment to assess patient concerns, including telephonic triage and symptom evaluation
  • Evaluate patient responses to care and adjust plans accordingly
  • Provide direct patient care, including:
    • Assistance with activities of daily living (ADLs), positioning, transport, and treatments
    • Safe use and transport of medical equipment
    • Monitoring for and responding to acute changes in patient condition
    • Administration of medications, including blood and blood products, in accordance with policy and best practices

Professional Relationships & Communication

  • Build and maintain professional, therapeutic relationships with patients, families, nurses, physicians, and other healthcare professionals
  • Partner with patients and families as active participants in care, consistent with Swanson’s Theory of Caring
  • Educate patients and families about diagnoses, treatments, and preventive care strategies
  • Communicate clearly and compassionately to support informed decision-making
  • Collaborate with the interdisciplinary care team to ensure quality, continuity, and coordination of care
  • Support and participate in community-based initiatives aligned with organizational values

Clinical Leadership & Professional Practice

  • Demonstrate professional conduct in alignment with:
    • Unit policies and procedures
    • Practice guidelines relevant to the clinical setting
    • The ANA Code of Ethics
  • Function as a responsive, reliable, and collaborative team member
  • Serve as a nurse educator and resource to patients, families, and colleagues
  • Support regulatory compliance and ongoing readiness initiatives
  • Achieve and maintain required certifications, including ACLS, per departmental standards
  • Advocate for patients within the scope of professional practice and legal standards
  • Promote a learning environment based on Benner’s Novice to Expert model, supporting recruitment, retention, and professional development

Minimum Qualifications

  • Graduate of a state approved and/or accredited School of Nursing BSN preferred
  • BLS certification with American Heart Association
  • Acute care medical-surgical experience

Pay Range

$37.00 – $95.37
