How you’ll make an impact in this role

Partner with clinicians during Medicare Annual Wellness visits to deliver comprehensive, preventive care for up to 8 patients per day.
Advocate for patients by providing clear health education, connecting them to vital community resources, and proactively resolving their care concerns.
Execute pre-visit planning and preventive screenings to ensure patients receive timely, accurate follow-up care aligned with wellness guidelines.
Manage population health initiatives within ACO guidelines to improve long-term health outcomes for the Medicare patient community.
Document clinical interactions with precision, ensuring all services meet CMS and compliance guidelines to maintain high-quality care records.

Minimum Requirements

  • Licensure / Certification / Registration: Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.
  • BLS Provider preferred. American Heart Association or American Red Cross accepted.
  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

Additional Preferences

  • 8 years of experience in Nursing preferred.
  • Skills: familiar with excel/google sheets, attention to detail, organization, and time management.
