About the role

As a Registered Nurse at Hackensack Meridian Health, you will function as a caregiver for a specific patient population to achieve desired outcomes. This position focuses on expanding knowledge and skills, providing effective direct care as part of the interdisciplinary team to a variety of complex patients. You will direct and oversee the care provided by other caregivers through delegation, validation, evaluation, and coordination, while effectively coordinating patient care and unit activities among nursing peers, physicians, and support services.

Responsibilities

  • Deliver nursing care according to the most up-to-date evidence-based research, nursing policies, and standards of care to ensure safe and excellent individualized patient care.
  • Assessment, Planning, Implementation, and Evaluation:
    • Collect comprehensive data pertinent to the patient's health or situation.
    • Perform age- and disease-specific assessments independently and systematically, focusing on physiologic, psychological, and cognitive status.
    • Recognize clinical and diagnostic changes in patient status.
    • Identify patient/family education needs based on age, disease, and culturally specific learning differences.
    • Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
    • Formulate patient/family-centered goals and outcomes based on assessment data.
    • Develop and coordinate a prioritized, patient/family-focused plan of care encompassing the continuum of care.
    • Communicate, coordinate, and implement the plan of care with the patient, family, and healthcare team.
    • Provide ongoing communication regarding the patient's status, care, and treatment plans.
    • Implement age- and disease-specific care in an organized and timely manner.
    • Provide or coordinate patient/family education and ensure understanding.
    • Perform care according to established protocols, including communication and documentation.
    • Provide nursing care and treatments reflecting patient needs, advocacy, and evidence-based practice.
    • Perform continuous Point-of-Care electronic documentation of assessments, plans, interventions, and evaluations.
    • Administer medication using bar-scanning and documentation technologies for safe delivery.
    • Coordinate multi-disciplinary discharge planning to ensure safe transitions and continuum of care.
    • Evaluate progress toward attainment of outcomes.
    • Utilize critical thinking skills to achieve desired patient outcomes.
    • Initiate interventions for changes in patient condition and unexpected responses to care.
    • Identify and address ethical issues in practice settings.
    • Enhance patient satisfaction by responding to opportunities for improvement.
    • Maintain required competencies, certifications, and continuing education.
    • Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
    • Perform physical requirements: lift a minimum of 35 lbs., push/pull a minimum of 10 lbs., and stand a minimum of 6 hours per day.
    • Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
    • Perform other duties and projects as assigned.

Requirements

  • A minimum of an Associate in Nursing or diploma in Nursing degree.
  • BSN in progress and/or willingness to acquire within three (3) years of hire.
  • Current and valid Registered Nurse license for the assigned HMH location by the date of hire.
  • BLS/CPR certification by the American Heart Association required within 60 days of hire.
  • Six (6) or more months of relevant nursing experience or active participation in Hackensack Meridian Health's Nurse Residency Program.
  • Ability to learn quickly, adapt to changing patient needs, and demonstrate a strong sense of accountability for improving patient and caregiver lives.
  • Exceptional focus on teamwork and dedication to ongoing education.
  • Passion for delivering high-quality care based on patient focus.
  • National Certification in area of specialty preferred when eligible.

Pay

Minimum starting rate of $45.20 per hour. The posted rate is a reasonable good-faith estimate of the minimum base pay for this role and may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, budget, and grant funding. Some roles may also be eligible for performance-based incentives, bonuses, commissions, or shift differentials for evening, night, or weekend shifts.
